IMPORTANT: If you are getting your passport for the first time DO NOT go to a website, other than www.travel.state.gov, to obtain passport information. You NEVER pay fees to anyone other than the US Department of State and that MUST be in the form of a check or money order. Fees are NEVER collected prior to your passport appointment. 1. SCHEDULE YOUR APPOINTMENT ONLINE SCHEDULING TOOL
2. COMPLETE A DS-11 APPLICATION FORM. These can be obtained from our office or online CLICK HERE .
3. PROOF OF CITIZENSHIP – A CERTIFIED COPY OF BIRTH CERTIFICATE issued by Department of Vital Statistics in the state the applicant was born, a previously issued passport, or a Certificate of Naturalization. Children under 16 must provide a Birth Certificate. Click HERE for Oregon Birth Certificates. Go to www.vitalchek.com for all states.
4. PASSPORT PHOTO - taken within the last six months. Photo size 2” x 2”, taken against a plain white or off-white background. Click here for information on Digital Photographs (We are able to provide you with a digital passport photo for $5.00, beginning February 1, 2025). Passport Photo services are available in our office for $20.00**.

5. CURRENT IDENTIFICATION – Such as a Driver’s License (not an ID card) or other acceptable I.D. Refer to reverse side of passport application. Children under 16 have Special Requirements.
